Disability Care and Support Pty Ltd trading as Borealis Care (ABN 77 616 062 616) is committed to protecting your privacy. We handle personal information in accordance with the Privacy Act 1988 (Cth), the Australian Privacy Principles, the NDIS Code of Conduct and the NDIS Practice Standards.

What we collect

Contact details; NDIS participant number and plan details; health and disability information needed to deliver safe support; emergency contacts; information about your preferences, goals and supports; and feedback you give us. Through our website we collect only what you enter into a form.

How we use it

To plan and deliver your supports, claim payment under the NDIS, meet our legal and reporting obligations, keep you and our workers safe, and improve our services. We do not sell personal information or use it for marketing without your consent.

Who we share it with

Only with your consent or as required by law: the NDIA and NDIS Quality and Safeguards Commission, your plan manager or support coordinator, other providers involved in your care, and emergency services where there is a serious risk to life or safety.

Storage and security

Records are stored in secure, access-controlled systems hosted in Australia. Staff receive privacy training and only access information they need for their role.

Your rights

You can ask to see or correct your information, withdraw consent, or make a privacy complaint by contacting us at info@borealis.com.au or 1300 936 347. If you are not satisfied with our response you can contact the Office of the Australian Information Commissioner at oaic.gov.au.
